Examples on how to fill out forms

The following are examples on how to fill out forms.  Each classification made (group) with its CV-value is summarized with other groups to give the daily CV.
The daily values are summarized by the end of the month, in order to derive the monhly average.

The next example is the same form as the On-Line Observation Form, which is rather simple and easy to fill out (if other method of submittal are preferred; see below).  This form goes to CV-Helios Network's mailbox and automatically transfers it to the assembly sheet for the proper month.

Methods of submitting your CV-observations:

1)   On-Line Observation Form (example as above, On-Line to CV-Helios Network mail
2)   E-mail as plain text or as attachment (preferred: Excel or Text, Word)
3)   Typed or handwritten letter by regular mail, to:
